How to access the service:
To access our service you can complete a Self-referral form online or you must have a referral completed by another healthcare professional

We are unable to accept self-referrals to physiotherapy if you are under the care of a consultant. If you are under the care of a consultant and need physiotherapy please contact your GP to complete a referral. For more information, please click on link to the FAQ leaflet below:
