Complex Care Team
Complex Care Nursing Staff offer a bespoke client specific nursing service 24 hours a day x 7 days a week in a community setting. The aim of the Complex Care Nursing service is to provide a holistic collaborative and co-ordinated approach to care for the person and their carer in order to maintain them in their own home. The Complex Care Team will act as an educational resource for other professionals striving towards improved, evidence based management of complex conditions in Health & Social Care and Educational setting.
The Complex Care Nursing Service enables those affected by complex health needs residing in the community. We develop an individualised care plan and provide specialist trained nurses to deliver care in the individuals own home, day-care and or respite facility. Complex Care provide education to clients/families/carers on the condition and how they can appropriately respond to the individual needs and have the confidence to continue to provide care.
Currently the team are expanding to include Enhanced care to provide care at home and prevent acute hospital admissions and affect earlier discharge.
