The Home Treatment Team provide specialist support and short term intervention for adults with a learning disability who present with mental ill health or behaviour which challenges them and their carers enabling them to stay in their own homes and avoid unnecessary admission to acute psychiatric hospital where possible.
The Home Treatment service recognises the importance of current support networks and the benefit of receiving support in a familiar environment, with minimal disruption to the individual’s daily routine.
Home treatment is a short term intervention which can be provided in a range of settings and offers an alternative to inpatient care. We aim to provide safe and supportive intervention to service users, carers and the wider public, whilst working with other statutory, community and voluntary agencies.

Description of services/opportunities provided
- Provide support to service users and carers following a crisis or to prevent a hospital admission.
- Provide support and treatment in the service user’s home following discharge from hospital for a planned period of time.
- Planned in conjunction with the Multi-Disciplinary Team.
- Early intervention – provide support and treatment to prevent admission

Referral Criteria
- Age range 18+
- Confirmed Diagnosis of Learning Disability
- Open case to Adult Learning Disability Community Team
- Resident from The Southern Health and Social Care Trust area

IPC guidelines in place including social distancing measures , wearing full PPE and wellness calls completed to service users prior to arranged visits
