This is a new services and was established to work with service users whose care has been delayed as a result of limited community placement or difficulties with the process of transfer to the community.
The purpose of the bespoke pathway/enhanced domiciliary care package is to ensure an agreed understanding across the Division and that placements are progressed within agreed timescales and the desired outcomes and benefits to the clients and carers achieved. This also includes step down from hospital, prison or returning ECRs.
The Bespoke Placement Officer will work with the Multidisciplinary team to embed the process of care management and progress cases within agreed timescales. This includes reviewing the assessments available, prioritising the service user needs in keeping with fundamental theories for the development of person centred lifestyles ensuring positive risk management and increased social opportunities using least restrictive practices.
The service aims to:
- Ensure positive risk management is followed
- Ensure evidence based practice is used with a value-led approach
- Aims to enhance an individual’s quality of life
- Aims to increase the individual’s repertoire of adaptive behaviours
- Increases the range of positive life opportunities for individuals
- Rejects the use of punitive interventions/restrictive practice
This is a pilot service and will be reviewed when Care Management review is completed.

Referral criteria
Referrals must be processed firstly through the Disability Division Accommodation Panel with relevant assessments in place including NISAT Core Assessment, PQC and Careplan. The outcome of this process will determine the need for specialist bespoke accommodation and will be taken forward by the Specialist Placement Practitioner. Where the clients have high support needs the case will transfer from the current Case Manager to the Specialist Placement Team to work intensively with the case and undertake the placement and review. When settled the case will transfer back to the relevant core community team.
Criteria for Service
- Service user must be an open case to Community Adult Disability Team
- Aged 18+
- Confirmed diagnosis of Learning Disability
- Need to plan for Complex Cases in transition from age 16/17 years old in conjunction with Children’s Services and Transition Team
- Client assessed as having
– High care/support needs
– Not suited to group living
– Present a risk to others
– Requires 24/7 care/support
