Complex Care Nursing Staff offer a bespoke client specific nursing service 24 hours a day x 7 days a week in a community setting. The aim of the Complex Care Nursing service is to provide a holistic collaborative and co-ordinated approach to care for the person and their carer in order to maintain them in their own home. The Complex Care Team will act as an educational resource for other professionals striving towards improved, evidence based management of complex conditions in Health & Social Care and Educational setting.
The Complex Care Nursing Service enables those affected by complex health needs residing in the community. The service will provide specific care information, support and advice from the age of 18 years or at onset of Brain injury. We develop an individualised care plan and provide specialist trained nurses to deliver care in the individuals own home, day-care and or respite facility. Complex Care provide education to clients/families/carers on the condition and how they can appropriately respond to the individual needs and have the confidence to continue to provide care.
Currently the team are expanding to include Enhanced care to provide care at home and prevent acute hospital admissions and affect earlier discharge.

Description of services/opportunities provided
The Complex Care Team will act as an educational resource for other professionals striving towards improved, evidence based management of complex conditions in Health & Social Care and Educational setting.
This can include:-
- Enteral feeding care and support such as peg and gastrostomy tube.
- Naso-gastric intubation
- Tracheostomy and respiratory care and support
- Colostomy and ileostomy care and support
- Cystostomy
- Intermittent urinary catheterisation
- Supports with Dialysis
- Wound management
- Client specific care based on assessed need
- Epilepsy care and support.

- Referral via PARIS for community.
- Hospital referrals via social work/ referral forms

Referral criteria
The Complex Care Nursing Team will provide care and support to adults with a disability who are aged 18 years and reside in the Southern Trust area and live in a community setting.
