Transitioning from Children’s Services

Young People transitioning from Childrens Services to Adult Disability Services will be accepted to the  core Adult Community Disability Teams (Physical Disability Team / Adult Learning Disability Teams) where a multi-disciplinary team consisting of Nursing / or Social Work Keyworkers, Occupational Therapists, Physiotherapists and Speech and Language Therapists will support the young person and their carer.

This Multi Disciplinary team will work in partnership with Children’s Services to support a smooth transition to Adult Disability Services, until the young person reaches 18 years of age.
