Who can access the team:
You are entitled to an assessment of need under the Health and Personal Social Services Order 1972 if you are:
- Aged over 18 and under 65.
- Have a physical disability as defined in the Disability Discrimination Act.
- Live in the Southern Trust.
We will work together with you to:
- Improve your independence and promote social inclusion.
- Improve choice and control in your daily life
- Enable you to access other support services

Who can refer to the team?
- Referrals can be from any health professional including your GP.
- Social Work self-referrals may also be accepted through the Social Work Duty System.

Who is in the team?
Administration
Our admin staff will be your first point of contact with the team and will be happy to deal with your enquiries.
Social Worker
Your social worker will be responsible for undertaking a holistic assessment of need and then co-ordinating supports to help meet your identified need. The assessment may identify a combination of supports from family, community, voluntary and statutory services. Support may also include therapeutic intervention, crisis management and emotional support.
The social worker will agree with you your required level of support and they will plan how this will be provided. They will review you social care needs and services at least annually.
Physiotherapy
Physiotherapists help people affected by injury, illness or disability through movement, exercise, education and advice. Physiotherapy is a science-based profession and takes a ‘whole person’ approach to health and wellbeing, which includes the patient’s general lifestyle. At the core is the patient’s involvement in their own care, through education, awareness, empowerment and participation in their treatment. The Physiotherapist will perform a comprehensive physical assessment before designing an individual treatment programme. The programme may consist of advice to a client on how to manage a condition through movement and exercise. This will help achieve an optimum level of physical strength, flexibility and function to promote independence.
Occupational Therapy
Occupational Therapists believe that actively participating in everyday life is essential to people’s health and wellbeing. We will help you to look at your priorities, what is important to you and want you to achieve in your everyday life.
OT can work with you:
To find different ways of completing everyday tasks to keep independent and active.
Advise you on adapting your home.
Advise on equipment to help you complete task
