The Trust provides day care places for older people over 65 years old who have a level of dependency arising from illness, disability, sensory impairment, or dementia and/or assessed as being at risk or vulnerable to risk. There are places on specified days specifically designated for dementia care or for those older people who require a rehabilitation service.
Accessing the service
Following a referral to the local social work team for older people, an assessment of the needs of the person for
whom the service is requested will be undertaken by a social worker or healthcare worker from the Memory Service team. During the assessment, information will be provided about the services available and any charges involved.
